SALARY RANGE: Hiring Range $21.65-$23.42 Per Hour.

Summary

Rural Resources Community is seeking a Care Coordinator.  This is a Full time, non-exempt position based in our Colville office working with clients throughout Stevens County.

Salary Information  

Our Care Coordinator position is offered at $21.65 to $23.42 per hour.  Actual wage will vary depending on factors, including but not limited to, experience and qualifications.  Employees who successfully meet or exceed performance expectations during their annual performance evaluation will be eligible for a 4% wage increase.

The full salary range for this position is $21.65-36.06 per hour and represents the low and high end of the total salary range for this position.

Duties & Responsibilities

At RRCA, we are dedicated to assisting people and communities in developing the skills, resources, and services necessary to improve their general welfare and we need a Care Coordinator who shares that passion.

In this key role you will be responsible for:

  • Work with individuals and their families to assist them in identifying and accessing supports.
  • Provides health education and uses motivational interviewing techniques to build engagement in goals and increase resiliency.
  • Utilizes health promotion, wellness coaching and awareness of chronic health condition to support client’s continued health and improvement.
  • Works to facilitate a reduction in utilization of emergency services and hospitalizations.
  • Maintains strict confidentiality and accurately documents work.

Education, Skills, and Abilities

Education and Experience

A Bachelor of Arts/Science degree in a human service-related field, two years’ experience working for a human services agency

or

Any combination of education and experience that would provide the applicant with the desired skills, knowledge and abilities required to perform the job.
