SALARY RANGE: Hiring Range $21.65-$23.42 Per Hour

Summary

Rural Resources Community Action needs a Care Coordinator. This is a full-time position in Republic, WA/Colville, WA. As a Care Coordinator, you will provide care coordination services to eligible individuals and their families, assesses needs, develops and monitors the implementation of service plans, makes appropriate referrals, and serves as a client advocate with other service providers. You will also work to facilitate a reduction in the utilization of emergency services and decrease the recurrence of hospitalizations. While using health education and motivational interviewing techniques to build engagement in goals, increase resiliency and build a pathway for continued health improvement and a healthy lifestyle.

Salary & Benefit information

The hiring range for this role is $21.65-$23.42 per hour. The full salary range of $21.65-$36.06 per hour represents the low and high end of the total salary range for this position. Actual salaries will vary depending on factors including but not limited to experience & qualifications. After the successful completion of our six-month training period, you will receive a 4% step increase.  As a regular employee, you may receive an additional 4% step increase on your annual anniversary with RRCA. Duties & Responsibilities

A Care Coordinator will need knowledge of community-based human services principles and practices, modern principles and practices of management and supervision, and federal, state, and local policies, procedures, and regulations applicable to program areas. With the ability to develop and implement short and long-range plans to meet program needs, establish and maintain effective working relationships with staff, outside organizations, and the general public, and perform social and economic research and development.

Education, Skills, and Abilities

To apply for this position, you would need a Bachelor of Arts/Science degree in a human service-related field, two years of experience working for a human services agency, with one-year supervisory experience, or any combination of education and experience which would provide the applicant with the desired skills, knowledge, and abilities required to perform the job.
