HOURS: 40 Hours Per Week
SALARY RANGE: Hiring Range: $19.87-$21.49 or$ 21.65-$23.42
Summary
Our Case Manager position is offered at $21.65 to $23.42 per hour. Case Manager Trainee is offered at $19.87 to $21.49 per hour. Actual wage will vary depending on factors, including but not limited to, experience and qualifications. Employees who successfully meet or exceed performance expectations during their annual performance evaluation will be eligible for a 4% wage increase.
Duties & Responsibilities
- Complete the initial interviews with clients and their families to assess the customer’s eligibility for services.
- Gather relevant and useful data from the client, family, other agencies, to formulate a psychosocial assessment of the client and his or her family.
- Complete the necessary paperwork to maintain documentation of client progress and adherence to the plan by all concerned.
- Provide regularly scheduled in-home monitoring of assigned clients.
- Remain current on state and federal policy, requirements, laws and regulations governing programs and services.
Education, Skills, and Abilities
Per Washington State Aging & Long-Term Support Administration Policy, Case Managers will meet at least the following minimum education and experience requirements:
