OFFERED SALARY RANGE: $21.65-$23.42 per hour.

FULL SALARY RANGE:  $21.65 - $57.73 per/hr. (Represents the low and high end of the total salary range for this position)

The full salary range representing the broader compensation band for the position and progression within that range is based on employee longevity and continued service. Position Summary

The Health Home Care Coordinator provides comprehensive care coordination services to eligible individuals and their families. This role involves assessing member needs, developing and monitoring individualized service plans, making appropriate referrals, and advocating on behalf of members with other service providers. Care Coordinators maintain a dedicated caseload and ensure consistent monthly engagement with assigned members across various settings. Health Home Care Coordinator’s support members in identifying and accessing resources, delivering health education, and applying motivational interviewing techniques to foster goal achievement, resilience, and healthy lifestyle choices. The Care Coordinator promotes wellness through coaching and awareness of chronic health conditions, aiming to reduce emergency service usage and prevent hospital readmissions.

Essential Job Functions

• Manage a caseload of clients while overseeing the delivery of care coordination services, ensuring compliance with Health Home program standards.

• Conduct comprehensive assessments, develop individualized Health Action Plans (HAPs), and monitor progress toward health and wellness goals.

• Coordinate with healthcare providers, community organizations, and other stakeholders to ensure integrated and client-centered care.

• Conduct in-person visits with clients across designated service areas, requiring frequent travel by vehicle. This includes driving to client homes, community settings, and partner agencies to perform assessments, deliver care coordination services, and facilitate referrals.

• Review documentation for accuracy, completeness, and compliance with HCA, MCO, HIPAA, and agency policies; support continuous quality improvement efforts.

• Performs other duties as assigned.

Minimum Qualifications

A Bachelor of Arts/Science degree in a human service related field with two years

experience working for a human services agency.

OR

Any combination of education and experience which would provide the applicant with the

desired skills, knowledge and abilities required to perform the job.

Required Certification and Licenses:

• WA State certified Health Home Care Coordinator (completed 2-day HCA training).

• Valid driver’s license in state of residence, or ability to obtain prior to employment.

• First Aid/CPR certificate (not required, but preferrable)

• Must have reliable transportation.

• Must maintain automobile insurance as required by the State of Washington,

• Acceptable completion of a criminal history background check.
